Mr. Miller,

   My experience was with the CB03, specifically. Just as your results state in the first paragraph, the CB03 does not work well at all with the Square D QO series breaker. And Square D products and QO series breakers are common in this area.

   I worked a year and a half long lighting and fire alarm refurbishment project in the Launch Control Center for complex 39 at Kennedy Space Center, Space Shuttle Program, and we had many LOTO’S in place during this project. The CBO3 worked well all single pole breakers EXCEPT Square D QO series. Unfortunately not all of our crew was aware of this problem and they were installed on QO breakers, failing miserably, sometimes even falling off on their own from the weight of the tag and lock, and the weight was nominal. (I used the smallest Master “luggage” lock I could find).

   This embarrassed us on more than one occasion in front of Government Inspectors in an extremely high profile environment. The CB03 should NEVER be used on a Square D QO series breaker.
